April 2025 – Present (Since Physical Launch)

Since launching in April 2025, 365 We Thrive has delivered integrated, community-based programming designed to strengthen community safety, stabilize families, expand economic opportunity, and elevate youth leadership across underserved Chicago communities. Our model addresses social determinants of health by integrating stabilization, workforce development, educational engagement, and community-based leadership cultivation to support long-term social and economic mobility.

Community Safety

2410 youth and families directly served through 365 We Thrive’s 2026 programming, community outreach, and violence prevention efforts—including MOVE • CONNECT • THRIVE, summer programming, REAL TALK CO., workshops, Community Impact Labs, and other collaborative events. The experiences were designed to foster youth voice, leadership development, create a sense of belonging, and connect participants to continued support. Community Impact Labs especially strengthen multigenerational cohesion and promote healthy, safe and peaceful interaction and physical activity in communities experiencing elevated risk factors. Real-time resources across social determinants of health were made available, with coordinated follow-up support to help address identified needs and foster stability.

40 conflict mediations facilitated to de-escalate disputes and prevent potential violence, addressing social media–driven conflicts and in-person tensions within the community.

Proactive Engagement: Promoting safer community environments through structured conflict resolution and proactive, relationship-centered engagement.

Family Stability & Housing Support

182 families stabilized, including:

142 resumes and job-skills training delivered to youth and adult participants resulting in noted economic empowerment
28 households supported through housing stabilization services
12 individuals supported through expungement navigation

Sustained Mobility: Strengthening long-term household security, economic mobility, and pathways toward self-sufficiency.

Economic Empowerment

48 workforce placements secured for youth and adult participants

Career Pathways: Transitioning participants from crisis to employment readiness, skill building, and sustainable career pathways.

Educational Outreach

835 students received direct educational resource support across school-based outreach initiatives and community impact lab distribution, strengthening academic readiness and school engagement. These efforts are supported by our STEP INTO SUCCESS™ campaign.

Crisis & Seasonal Support

250 essential winter items distributed through the “Share the Warmth” initiative, reducing seasonal hardship and reinforcing family stability.

Youth Leadership & Civic Engagement

121 participants engaged in structured leadership development, resulting in:
• 6 youth-launched businesses
• 23 emerging community service leaders
• 30+ community impact initiatives advancing neighborhood revitalization and civic participation

Program Effectiveness

98% of evaluated program participants reported positive gains in confidence, knowledge, connection, leadership, and/or readiness for education or career next steps.

Impact data is collected through participant surveys, attendance tracking, and program completion benchmark.

Early outcomes and community engagement continue to inform program growth as 365 We Thrive strengthens partnerships and expands capacity to meet increasing community needs.
